What to Bring

Patients are reminded to please bring the following:

• Your doctor’s referral
• Previous X-Rays, CTs and/or Ultrasounds of the same region
• Your Medicare and/or Veterans’ Affairs Card

If your examination relates to a Workers Compensation Claim or Third Party Insurance Claim, please bring the following information with you:-

• Date of injury
• Employer’s Name & Address, and Contact Name & Telephone Number at your workplace (if Workers Compensation Claim)
• Insurance Company Details: Name & Address of insurance company
• Claim Number
• Claims Manager & his/her contact number.
